SMI service
The SMI Physical Health Checks service is a three-way partnership with Haringey GP Federation (and member practices), BEH Mental Health Trust and Mind in Haringey. This service offers an annual physical health check with a view to highlight any potential physical health issues as well as offer holistic support for lifestyle factors. Appointments are offered within practices, in care homes/support accommodation, as home visits and also offering telephone consultations where clinically needed.
Social Prescribing
We have a proactive social prescribing service operating within our PCN practices. Through hiring Social Prescribers, the PCN is contributing to the increasingly personalised and changing landscape of primary care.
Social Prescribing helps to empower people when taking steps to reach their goals and co-produce a personalised well-being plan that focuses on ‘what matters to me’. Social Prescribers take a holistic approach to people’s health and wellbeing when connecting and signposting people to community groups and statutory services for practical and emotional support.
How Social Prescribers can help:
· Signpost services with regards to benefits, debt management and housing issues
· Connecting to local befriending or community groups
· Volunteering and assistance to work schemes
· Be more active or increasing healthy eating
· Joining in with local groups like exercise classes or gardening groups
Referral Criteria
· Must be over 18 years old
· A patient at a GP Surgery in our PCN
· Adults with mild to moderate long-term mental health problems
· Social needs which affect your health and wellbeing
· Patients who are lonely or isolated
· Patients who frequently attend either primary or secondary healthcare
Learning Disability reviews
Those patients aged 14 and over on our practice PCN learning disability registers are being offered an annual health check each year.
Allotment Patient Group
Patients within our PCN practices can join Arcadian Gardens Surgery Allotment page here. The idea is to provide a therapeutic outdoor space that promotes gentle gardening exercise, healthy eating, and community connection.
